Responsible for the delivery of appropriate, timely and beneficial care for customer-owners which promotes quality and cost-effective health care outcomes working within established standards for ca! se management practice, and Advance Access principles.
KNOWLEDGE, SKILLS, AND ABILITIES:
Creates, develops, and nurtures culturally appropriate interactions and connections with each other, customer-owners, and the community. Consistently models appropriate behaviors, attitudes and skills that support full customer ownership of all health issues. Mentors RNs and other staff members on developing effective customer relationships Demonstrates and facilitates exemplary customer service and satisfaction. Shares and receives information, opinions, concerns, and feedback in a supportive manner. Works collaboratively by building bridges, and creating rapport with team members within departments and across the organization. Models effective communication skills with internal and external customers. Communicates effectively with staff members in different forums using multiple mediums. Formally mentors other staff members on team building and communication skills! . Communicates with staff on a regular basis to illicit issues! , questions, training needs or best practices. Coordinates teambuilding opportunities for nursing, support staff, and clinical teams as appropriate. Makes meaningful improvement to services, program, and processes and/or organizational effectiveness that creates new value for customer-owners and employees. Participates with department or unit leadership to identify appropriate clinical and process improvement goals. Assists with department, division and corporate-wide quality projects as needed. Actively mentors other nurses and support staff to build clinical, leadership and communication skills. Excellent understanding of SCF improvement tools and demonstrated ability to use them in their own practice as well as mentor others in their use. Seeks out additional learning opportunities to continue to develop the technical and professional skills needed now and in the future. Takes responsibility for all work activities and personal actions by following through on commitments! . Demonstrates leadership, communication and teaching abilities consistent with guiding and facilitating learning about SCF processes and principles. Works in a mentoring role to further develop professional and leadership skills. Consistently seeks professional development opportunities for self and team. Ability to perform job using theory-based nursing practice model, evidenced based practices, and shared governance in a multidisciplinary team model. Familiar with and able to apply SCF Core Concepts in their work. Respected by co-workers in serving our customers with quality care, same-day access and timely care that follows our âdo todayâs work todayâ philosophy. Ability to participate in a multidisciplinary clinical team. Demonstrated skill in ensuring effective communication and exceptional customer service. Knowledge of clinical quality improvement models and process. Knowledge of scope of customer-owner care services provided by other clinical service lines an! d the referral process. Knowledge of clinic practices and ability to ac! t as a resource for clinical and non-clinical personnel and assist the providers as necessary. Demonstrated proficiency in applying the operational principles and the primary care model in own clinical team as well as the ability to inspire and share with other teams and clinics.
QUALIFICATIONS:
SCF programs are established for the purpose of serving a primary population comprised of Alaska Natives who are affiliated with the Cook Inlet Region Inc. (CIRI), and Alaska Natives and American Indians within SCFâs geographical service area. Employees should have a thorough understanding of the cultures and the needs of the CIRI members, and of the general culture and needs of Alaska Natives and American Indians. Such knowledge is critical in order to ensure the achievement of the mission and vision of SCF: working together with the Native community to provide the best services with Native staff who value the family and family wellness as the heart of the Nat! ive community. 1. Graduate of an accredited school of nursing. Bachelorâs degree or higher level educational program in health and human services is preferred. 2. Current registered nurse license in the State of Alaska. 3. Basic Life Support (BLS) certification is required. ACLS, NRP, ATLS, ALSO and PALS as required for specialty. 4. Two (2) years nursing experience at SCF or two (2) years RN supervisory or management experience. 5. Willing to act as clinical nurse specialist in chronic disease for primary care. 6. Meets all requirements for RN Case Manager I-II.
