Providence is calling a Primary Care Licensed Practical Nurse to the Providence Transitional Care Center in Anchorage, AK.
In this position,! you will:
Provide professional nursing care to a specific group of residents in a long term care environment
Work in a challenging environment assisting with clinical interventions and resident activities of daily living
Be accountable for the supervision of team members, coordinating referrals to other members of the interdisciplinary healthcare team, families and physicians.
Serve as point of contact for resident information.
Be accountable for the delivery and supervision of safe, quality care that is individualized for their assigned patients and follows the nursing process in accordance with the Nurse Practice Act (assessment, planning implementation, evaluation).
Perform all duties in a manner that respects and supports the family centered care model.
Required qualifications for this position include:
Must be a graduate of an accredited/approved school of nursing as a Licensed Practical Nurse (L! PN).
Must be licensed as a Licensed Practical Nurse i! n the State of Alaska at time of hire.
Must be proficient in medication administration.
Able to clearly articulate and communicate ideas verbally and in writing.
Preferred qualifications for this position include:
Six months nursing experience in a health care facility preferred (acute care, long term care, rehabilitation center).
Acquired skill or ability to become proficient in IV therapy within scope of practice according to licensure.
About the Transitional Care Center
The Transitional Care has a total of 52 beds with a variety of health rehabilitation issues (ages 20 and up). This is an exciting, busy unit with constant change, many admits, discharges.
