Centra Healthcare Solutions is seeking an experienced Registered Nurse (RN) that is licensed or in the immediate proc! ess, and qualifies, of obtaining RN licensure in this respective state to work in the specialty area of Nurse Management - Home Health.
The Registered Nurse (RN) is responsible and accountable for the delivery and coordination of patient care and the application of the nursing process. The RN demonstrates the ability to make clinical judgments and utilize critical thinking in an effective and efficient manner with supervision. As well, the Registered Nurse (RN) will perform according to nursing standards and is accountable in managing patient care and assisting others in the management of patient care.
Requirements include:
Must have graduated from an accredited school.
Current state license as a Registered Nurse (RN) or willing to obtain licensure in another state, assuming that all pre-requisites have been satisfied.
2-5 years of recent Home Health Nursing experience highly preferred
2-3 years of Nursing management experience in a Medic! are certified Home Health agency with an average daily census ! that is more than 50 patients is preferred.
Management experience of a nursing unit or management experience in a Home Health agency is preferred.
Experience with Joint Commission or state survey regulations for Home Health is preferred.
Current CPR certification.
Current BLS card.
Duties and Responsibilities
Assesses patient needs, signs and symptoms indicating physiologic and psychosocial changes in the patient's condition
Utilizes the nursing process to assess, plan, implement and evaluate patient care
Collects, analyzes, and interprets data and information from health care members and documents actual and/or potential nursing diagnoses.
Documents the patient's plan of care using identified nursing diagnoses, expected patient outcomes, and selected nursing interventions.
Provides a safe, comfortable and therapeutic patient environment.
Initiates and evaluates patient and/or family education.
Revises and documents the plan o! f care according to evaluation, changes in medical plan of care, and effective/ineffective nursing interventions.
Monitors the safety and effectiveness of the environment and equipment.
