Southcentral Foundation (SCF) is located on the Alaska Native Health Campus. SCF is ! an Alaska-Native owned, nonprofit health care organization.
Our award-winning 'Nuka System' of case is based on customer-ownership and relationships, integrated care teams and traditional Native values.
The Primary Care Clinics integrated care teams consist of Provider, an RN Case Manager, CMA, and an administrative support staff member. These staff together care for an exclusive panel of customers. This integrated care team has the opportunity to build relationship with these customers over time and address their preventative, acute and chronic healthcare needs. The result is high continuity of care and interdisciplinary teamwork. Resources such as integrated behavioral health consultants and dieticians are available in clinic for referral and quick consultation.
SCF is seeking experienced Full time Nurses to act as RN Case Managers in the Primary Care Clinics

Must have a current RN licenses with (1) o! ne year of RN experience required. If you are an experienced RN Case Manager who welcomes adventure and is looking for a new challenge, join nationally recognized facility.
