Responsible for the delivery of appropriate, timely and beneficial care for customers, which promotes quality and cost-effective healt! h care outcomes working within established standards for Home Care. This position is a job progression, two levels, designed to provide progressively more responsible and independent work experiences to participants as part of the overall assessment of their ability and capability. Promotion between job levels is based on performance of prior assignments and the demonstrated ability to successfully handle more progressively responsible assignments.

Customer Care & Relationships:
Creates, develops, and nurtures culturally appropriate interactions and connections with each other, customer-owners, and the community.
Provides standard nursing care for customers throughout the age continuum.
- Develops, implements and evaluates care plans.
- Promotes health care outcomes with currently accepted hom! e care practice guidelines.
- Provides customer/family ! education, advice and information on health assessment, disease processes, medications and treatment plans and health maintenance.
- Assesses customer needs using established guidelines, protocols, and pathways.
- Assesses customerâs home environment including access, mobility, safety and infection control needs, includes identified needs in plan of care.
- Provides appropriate follow-up as directed or per established guidelines.
- Evaluates care plan and makes appropriate adjustments.
- Documents all aspects of care and education provided.
- Collaborates proactively with all interdisciplinary team members in Home Based Services (HBS), Alaska Native Medical Center (ANMC) and any other referral providers with a customer-focus to facilitate and maximize customer healthcare outcomes.
- Communicates care outcomes to Primary Care referral provider team and provi! des updates on customerâs status and progress. Assures that all care provided is covered by a physicianâs order.
- Participates in multi-disciplinary customer care conferences for high risk or complex customers as needed, within HBS and referral organizations.
- Maintains compliance with applicable laws such as customer confidentiality, Americans with Disabilities Act, Workersâ Compensation, abuse reporting, principles of consent, and advanced medical directives.
- Advocates for the customer/family at the service-delivery level and at the policy-making level fostering the customer/familyâs decision-making, independence, and growth and development.
- Utilizes best practice model to identify, incorporate or develop best practices for home care. Collaborates with other teams to share and establish best practice for home based services.
- Integrates factors related to quality, safety, efficiency, and cost effectiveness in planning, del! ivering, monitoring and evaluating customer care promoting the most eff! ective and efficient use of human and financial resources.
- Utilizes customer based documentation, scheduling, tracking system (MYSIS) to track, monitor, and ensure appropriate follow-up of customers. Utilizes RPMS to gather assessment data, obtain updates and track appointments.
Communications & Teamwork:
Shares and receives information, opinions, concerns, and feedback in a supportive manner.
Works collaboratively by building bridges, and creating rapport with team members within departments and across the organization.
Acts as a resource.
- Participates in department, ANMC campus, or corporate-wide teams and committees.
- Active member of clinical team, contributing to overall team function, team building and enhancement.
- Participates in the continued development of the role of Home ! Care as an extension and team member in the Primary Care-Advance Access model.
- Coaches and mentors all clinical team members to build or enhance capability and competency.
- Responds to emergency situations per SCF emergency preparedness plan.
Ability to participate in a multidisciplinary clinical team.
Improvement & Innovation:
Makes meaningful improvement to services, program, and processes and/or organizational effectiveness that creates new value for customer-owners and employees.
Knowledge of clinical quality improvement models and process.
Knowledge of effective teaching processes and principles in planning, providing and evaluating optimum health care.

Takes responsibility for all work activities and personal actions by following through on commitments.
Knowledge of quality management and improvement processes to comply with internal and external standards, i.e., Joint Commission for Accreditation of Hospital Organization (JCAHO).
Knowledge of general medical procedures and the ability to anticipate complications and the indicated therapeutic interventions.
Knowledge of Home Care practices and ability to act as a resource for Home Care personnel.
Proficiency in the use of appropriate computer based software and office equipment.
Knowledge of 1974 Privacy Act is required as use of customer record is an integral part of the position and privacy of individuals must be protected to the fullest.
Ability to maintain quality, safety and infection control standards.
Additional Responsibilities for Home Care Registered Nurse II: Meets all requirements of Home Care Register! ed Nurse I in addition to the following:

Ability to mentor and orient new staff members.

Knowledge and ability to apply Process Improvement theory and techniques to Home Care.
Workforce Development Skills & Abilities:
Knowledge and ability to apply Home Care and Nursing principles.
QUALIFICATIONS:
SCF programs are established for the purpose of serving a primary population comprised of Alaska Natives who are affiliated with the Cook Inlet Region Inc. (CIRI), and Alaska Natives and American Indians within SCFâs geographical service area. Employees should have a thorough understanding of the cultures and the needs of the CIRI members, and of the general culture and needs of Alaska Natives and American Indians. Such knowledge is critical in order to ensure the achievement of the mission and vision of SCF: working together with the Native commun! ity to provide the best services with Native staff who value the family! and family wellness as the heart of the Native community.
Graduate of an accredited school of nursing. Bachelorâs degree or higher level educational program in health and human services is preferred.
Current registered nurse license in the State of Alaska or ability to obtain Alaska license within thirty (30) days of date of hire.
Basic Life Support (BLS) certification is required. ACLS, NRP, ATLS, ALSO and PALS as required for specialty.
At least eighteen (18) years of age.
Valid Alaska driverâs license.
No more than 3 moving traffic violations (as defined by Alaska Statutes Title 28) or 2 at-fault traffic accidents in the previous three (3) years.
The driver must not have had a âDriving Under the Influenceâ (DUI) or âDriving While Intoxicatedâ (DWI) violation in the past five (5) years.
Additional Qualifications for home Care Registered Nurse II: Meets all requirements of Home Care Registered Nurse in addition ! to the following:
Certification in Case Management and/or other certifications pertinent to clinical settings.
Two (2) years experience as an RN.
Certification in lactation teaching for those nurses working in home based programs for families and newborns.
Successful completion of job progression checklist.
Working Environment:

- Additional physical demands: this position requires driving; employee may be required to work holidays, evenings, weekends, and nights; employee must be able to respond when needed by prompt appearance at a required location; employee may be required to provide customer-owner care in the customer-ownerâs home or in other locations away from employeeâs primary work site; travel may be required; the employee is able to work under physically and emotionally stressed conditions.
