Providence is calling a Registered Nurse (RN) Case Manager ( 1.0 FTE, Day shift) to Providence Family Medicine Center in Anchorage, Alaska.
The Family Medicine RN Case Manager works as a team member to support the operational needs of the clinic for daily patient flow and assumes responsibility for the ongoing case management of an assigned panel of patients.
In this position, you will:
Coordinate with an interdisciplinary team that includes the patients, families, physicians, medical assistants, social workers, counselors, and other FMC support staff to provide high quality care within the FMC model of a Patient Centered Medical Home (PCMH).
Advocate and support the case management process, matching an individual’s ongoing needs with the appropriate level and type of medical, psychological, health, or social care/services within the organization or across multiple organizations. Partner in identifying and achieving timely patient care needs with implementation of the plan of care that is appropriate, resourceful, and valuable as determined by the interdisciplinary care team, effectively utilizing tools that measur e, monitor, and manage quality and stewardship of patient care resources.
Performs triage of presenting patients to determine appropriate course of action when required.
Participates in an interdisciplinary care team.
Qualifications:
Required qualifications for this position include:
BachelorsDegree in Nursing required.
Licensed as a Registered Nurse (RN) in the State of Alaska
2 years professional nursing experience
Ability to work with a wide range of patients of varied socioeconomic and ethnic backgrounds
Ability to maintain records and charts and to prepare clear, concise reports
Preferred qualifications for this position include:
Bachelors Degree in Nursing (BSN)
Prior clinic or outpatient experience
Supervisory experience
